TRANS HIV NEEDS SURVEY
ALL WEEK LONG! APRIL 11 – 18
We worked with some amazing members of the Phoenix Trans Community to create this survey to establish a foundation for building future Trans-Focused programs to make meaningful impacts around HIV in our community!
If you identify as Trans or Non-Binary, we’d love to invite you to participate in this survey! We’ll use the information to seek funding and resources to help address the needs that are identified. We’ll also compile the information and make the results public in case others can use the information to help the Trans & Non-Binary Community.
Surveys are completely anonymous. No names, emails, phone numbers or addresses are collected.

About Clínica Liberación
Trans Queer Pueblo's Clínica Liberación is held once a month in partnership with PACH (Phoenix Allies for Community Health) and Peacework Medical to provide free primary care and Hormone Replacement Therapy (HRT) to the Greater Phoenix LGBTQ+ migrant community. About Trans Queer Pueblo
Trans Queer Pueblo is a racial and gender justice organization collectively led by 300+ LGBTQ+ undocumented, migrant and BIPOC in Phoenix. Our Health Justice Project fights for liberation and self determination by healing injustices and breaking the barriers of oppression in healthcare.

AZ Center for Change will also be on-site all weekend for you to meet! AZ Center for Change promotes positive change in your life through the use of established best practice models, compassionate treatment and professional collaboration. They offer specialized counseling services as well as general mental health counseling.
About SOUTHWEST CENTER
Southwest Center's TRANS Program includes peer health navigation, HIV resources, behavioral health counseling, and primary care, including hormone replacement therapy. Our Transgender Health Navigator also helps people navigate insurance coverage, changing names and/or gender markers on official documents and providing referrals to other resources available.
About Rose Medical
Rose medical is a private medical clinic with a focus on transgender medicine. We offer HRT with the latest and updated information on the best regimens. Rose Medical also specializes in facial aesthetic transformation via fillers, as well as offering hair regrowth therapy utilizing PRP therapy. Currently offering a flat rate cash pay with a sliding scale if needed, and currently accepting several insurance plans.
About Terros Health
The Turning the Tide (T3) program at Terros Health focuses on MSMs of color and Transgender individuals to provide Care Navigation, linkage to SUD and Mental Health services, HIV/HCV/STI testing, support groups, and more. Our Care Navigators work with each client 1:1 to link them to services and to set achievable goals to live their healthiest, happiest lives. Terros Health also offers showers, clothing exchange, meals, educations, and more for all clients we serve.
